Discharge Teaching and Medication Adherence
Medication adherence after hospital discharge is a common concern in adult nursing care. Some patients forget medication instructions because they receive a large amount of information before going home. A small hospital project used a discharge teaching checklist with 90 adult patients. Nurses explained dosage, timing, warning signs, and follow-up appointments before discharge. Two weeks later, many patients reported that the checklist helped them remember when to take their medication. However, the project did not measure long-term adherence. The findings suggest that structured discharge teaching may support safer medication use, but further evaluation is needed.
